In this second episode of Wolfenstein 3D, titled "Operation Eisenfaust," the main character, William J. Blazkowicz, needs to infiltrate the Nazi stronghold known as the "Castle Hollehammer". The fortress is heavily guarded, and Blazkowicz must fight his way through several levels, battling Nazi soldiers and officers along the way. Episode 2 takes place in Castle Hollehammer.

BJ did see mutants before, during the events of Spear of Destiny, part of the Nocturnal Missions. In these godforsaken events, leading up to Episode 1, BJ was captured attempting to capture plans for Operation Eisenfaust. Now, he finds himself fighting to end it all. BJ encounters plenty mutants with guns on their chests during this episode and they will serve as his most dangerous non-boss foes, as what one can expect from mutants. The boss, Dr. Schabbs, is the mastermind of the plan himself. Ultimately killing Dr. Schabbs ends the episode.
Blazkowicz's ultimate objective is to find and destroy the research laboratory responsible for developing a new kind of super mutant soldier. This lab is located deep within the fortress, and Blazkowicz must find his way through a maze of corridors and secret rooms to reach it. Along the way, he must also gather intelligence on the Nazi's plans for world domination and foil their attempts to create a powerful new weapon. The stakes are high, and Blazkowicz's mission is crucial in the fight against Nazi tyranny.
